Our approach is simple: We listen to our participants and their caregivers to truly understand their needs and desires.Level of Care AssessorJOB SUMMARY:The Level of Care (LOC) Assessor is responsible for conducting comprehensive, state-certified evaluations to determine whether prospective participants meet the nursing home level of care requirement for eligibility in the Program of All-Inclusive Care for the Elderly (PACE). This role involves performing in-home assessments, documenting clinical and functional status, identifying care needs, and collaborating with state agencies and the interdisciplinary team (IDT) to ensure timely, accurate eligibility determinations. ESSENTIAL DUTIES AND RESPONSIBILITIES: Conduct in-home, state-certified level of care assessments for prospective PACE participants.Accurately determine whether individuals meet the nursing facility level of care criteria as required by the State Department of Health and Human Services (DHHS).Coordinate and communicate effectively with state-level personnel to complete and document LOC determinations.Complete all required LOC certification and re-certification training as mandated by the state annually.Enter and manage participant evaluation data within the State's electronic system and internal records (e.g., EMR).Communicate assessment findings clearly to the Interdisciplinary Team (IDT), contributing to eligibility decisions and enrollment planning.Identify participant needs, health conditions, support systems, and goals to assist in initial care planning.Provide education to participants and families about PACE services and level of care requirements.Independently schedule assessments and follow-ups, demonstrating strong time management and prioritization.Complete monthly re-assessments or re-determinations as required.Participates in organizational quality initiatives by contributing to data accuracy, identifying process improvement opportunities, and supporting efforts to enhance assessment consistency and participant outcomes.Ensure documentation is accurate, timely, and aligned with regulatory and organizational standards.EXPERIENCE AND EDUCATION: Bachelor's degree in Nursing, Social Work, Gerontology, or other health-related field (varies by state; licensure may be required).Must hold and maintain active state certification to conduct nursing facility level of care assessments.Prior experience conducting clinical or functional assessments in a healthcare, long-term care, or community-based setting preferred.PRE-EMPLOYMENT REQUIREMENTS: All Employees - Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact. Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
